The programme assists frail aged, younger people with a disability and Carers to maintain independence in their own homes. The service provides short-term, "once off" or ongoing practical assistance with:
Minor home maintenance e.g. cleaning gutters, window cleaning, replacing tap washers etc,
Information and advice regarding home security and your personal safety,
Home support services tailored to your needs e.g. domestic assistance, social support

CHAP is funded by the State and Australian Governments to provide services to assist the frail aged, the younger disabled and their carers to remain living independently in their community. We achieve this by acting as a broker on behalf of the client and contract the work to independent contractors to provide the service in the client's home. Referrals can be made by the client's family, neighbour, doctor, hospital or other advocates with the client's permission or by the clients themselves.
