Community Options

Clients receive up to four hours of basic support per week depending on assessed need. These services may include domestic assistance (cleaning), personal care (showering) and social support (shopping/bill paying) to assist the person to remain at home.

CHAP is funded by the State and Australian Governments to provide services to assist the frail aged, the younger disabled and their carers to remain living independently in their community. We achieve this by acting as a broker on behalf of the client and contract the work to independent contractors to provide the service in the client's home. Referrals can be made by the client's family, neighbour, doctor, hospital or other advocates with the client's permission or by the clients themselves.
