Services provided through CHAP are either self-funded by clients or provided on behalf of other agencies. Specified services are as per contractual agreement and within CHAP service delivery parameters and resources.

CHAP is funded by the State and Australian Governments to provide services to assist the frail aged, the younger disabled and their carers to remain living independently in their community. We achieve this by acting as a broker on behalf of the client and contract the work to independent contractors to provide the service in the client's home. Referrals can be made by the client's family, neighbour, doctor, hospital or other advocates with the client's permission or by the clients themselves.
