CHAP’s goal is to positively impact on the lives of frail older people and their carers through our community care services. We are an accredited provider who aims to maintain its focus on assessed client need and locally based service delivery.

CHAP covers Gawler and surrounding districts, the Barossa Valley, Wakefield Plains, Lower North and Yorke Peninsula serving those who need varying levels of support to achieve their goal of independence.

We make available up to 100 Community Aged Care Packages as well as low level in home support to people through Home and Community Care. We are also an approved provider of Veteran’s Home Care.

We are "providers of quality community care" with over 30 staff and 350 registered independent contractors committed to ensuring a high level of assistance and care to our clients.
