Community Aged Care Packages

CACPs offer an integrated package of services for frail aged people. If your care needs are more complex, or you need a range of services like help with bathing, meals, shopping and getting around, there's a specially designed service that can help you: a CACP, which is an individually tailored package of care services, which helps people with diverse care needs to stay living at home by co-ordinating the different services they need.

You first have an assessment by an Aged Care Assessment Team. If a CACP is appropriate for your needs, you will be referred to a coordinator.

CHAP provides a package of care with negotiated hours per week depending on the individual client's assessed needs.

CHAP is funded by the State and Australian Governments to provide services to assist the frail aged, the younger disabled and their carers to remain living independently in their community. We achieve this by acting as a broker on behalf of the client and contract the work to independent contractors to provide the service in the client's home. Referrals can be made by the client's family, neighbour, doctor, hospital or other advocates with the client's permission or by the clients themselves.
