About our Programmes

CHAP acknowledges that our clients have individual needs which may change over time. We offer a wide range of programmes to ensure we are able to provide options, choices, and therefore an informed pathway of care. We work with our clients to tailor packages that meet their individual assessed needs, and as part of our service, we hold regular client reviews.

These services range from the HomeAssist programme providing domestic assistance and home maintenance, to our Community Aged Care Packages which meet more complex needs.
